Extra-Curricula Enrolment Form

You will be invoiced for 12 weeks at the time, however smaller payments can be arranged on request.

Terms and Conditions: Credit is given for missed classes only if all payments are made by the due date. Credit will not be given after any late payment.

Cancellation requires written notification to ensure you are not charged any unnecessary fees. Please note, there is a no refund policy unless Play Frog is unable to provide its services. Your enrolment assumes you agree with our terms and conditions.
